Skill Saturday: The 2013 guidelines for the Identification of a STEMI have changed: • A new or presumed new LBB is no longer an acceptable indication of a STEMI • EMS provides should use the Sgarbossa criteria to diagnose a STEMI (with presence of LBBB) • Isolated ST-depression in V1-V4 is an indication of a posterior STEMI • Widespread ST-depression with ST-elevation in aVR is an indication of proximal LAD/LMCA occlusion • Hyper acute T waves are usually an early indication of a STEMI Sgarbossa Criteria A: In the beat on the left, note the inappropriate concordance in a lead with a positive deflection of the QRS. Sgarbossa Criteria B: In the beat on the right, you'll note inappropriate concordance in a lead with a negative QRS. Inappropriate concordance of ≥ 1 mm is highly predictive of STEMI. Sgarbossa Criteria C: The middle beat shows a ≥ 5 mm excessive discordance and is the weakest criteria of the three; it may indicate a STEMI only 50% of the time. However, even lower sensitivity is still better than saying that it can't be done.
