Upon arrival you find this 20ish year old white male lying face down on the roadside. Responsive only to deep painful stimuli. As you role him you find a black handgun lying underneath him. Breathing appears labored. You cannot obtain any information from him. You find a bag of what looks like Potpourri falling out of his jacket pocket. Due to weather you only have 1 First Responder on scene, no other available. The small local Sheriff's dept is tied up on a standoff 15 miles away, closest State Police or LEO backup is 20 min away. Closest Level 2 is 10 min away, Level 1 is 15 min. You HAVE NO OTHER RESOURCES TO RESPOND!!!! HEENT- Burns around lips Pupils- 2 mm slow Neck- Slight JVD, Trach Mid line Thorax- as you see it. Lungs- Decreased on left, rails in left lower Back- Clr Abd- Appears normal Pel- Clr Extrem- Burns to fingers, Cyan in nail beds, nothing else noted Neuro- Unable to assess BP- 65/42 HR- 48 RR- 08 Shallow FSBS- Not assessed SpO2- 76% on NRB via FR What is your treatment plan going to be?, How will you handle the firearm issue? Seems At the Start of Every Summer I have several of these Cases. .............. Scythe The most common shock-related injury is a burn. Burns suffered in electrical incidents may be one or more of the following three types: Electrical burns cause tissue damage, and are the result of heat generated by the flow of electric current through the body. Electrical burns are one of the most serious injuries you can receive and should be given immediate attention. High temperatures near the body produced by an electric arc or explosion cause arc or flash burns. They should also be attended to promptly. Thermal contact burns occur when skin comes in contact with overheated electric equipment, or when clothing is ignited in an electrical incident. Internal Injuries Excessive electricity flowing through the human body can cause serious damage to internal organs. Resulting medical problems include hemorrhage (or internal bleeding), tissue destruction, and nerve or muscle damage. These internal injuries may not be immediately apparent to the victim or observers; however, left untreated, they can result in death. Involuntary Muscle Contraction Normal muscle contraction is caused by very small amounts of electricity that are created within our bodies. Muscles violently contract when stimulated by excessive amounts of electricity. These involuntary contractions can damage muscles, tendons, and ligaments, and may even cause broken bones. If the victim is holding an electrocuting object, hand muscles may contract, making it impossible to drop the object and prolonging contact with the current. Also, injury or death may result when violent muscle contractions cause workers to fall from ladders and scaffolds or inadvertently strike other objects. Scythe You off duty hiking with some friends, upon waling up a narrow ravine your friend who is a 21 year old white female slips down about 6-8 feet to a dry creek bed. She screams loudly and i crying. When you arrive at her this is what you find. Initially no pedal pulse. She screams "Don't touch it" The only other injuries is "road rash" to her hands and she is complaining of some mild lumbar pain. Your 3 miles into the woods, No Cell service, No sat phone, no radio. JUST YOU and 1 non-medically trained friend. You DID NOT bring your first aid kit. How are you going to package this and extricate her to the road 3 miles away where you can get cell service?
